Where PEX Sp. z o.o. processes personal data (refers to the database of pharmacies and companies that have personal data in their name, data of contractors, customers and persons participating in PEX consulting and research projects, and others – similar or related), hereafter “PEX”, informs you of:

  • full details of PEX, including contact details PEX Sp. z o.o., with registered office in Warsaw, post code: 02-796, ul. Migdałowa 4D lok. 46, entered in the Register of Entrepreneurs of the National Court Register kept by the District Court for the Capital City of Warsaw in Warsaw, 13th Commercial Division of the National Court Register under number 0000178089, holding NIP number: 951-20-93-564, holding REGON number: with share capital of PLN 12,200,000.00 paid in full in cash,
  • the appointment of a Data Protection Officer by the controller (contact: [email protected], + 48 22 886 47 15, or by letter to the controller).

 

  1. The administrator of the personal data is PEX Sp. z o.o. with its registered office in Warsaw (02-796), Migdałowa 4D lok. 46, entered in the register of entrepreneurs of the National Court Register under the number 0000178089.

  2. The controller and/or the Data Protection Officer can be contacted via e-mail at [email protected], by telephone at +48 (22) 886-47-15 or in writing to the address of the controller’s registered office.

  3. Detailed information on the processing of personal data including the purposes of the processing and their legal basis, the recipients or categories of recipients, the duration of data retention and the requirement to provide data can be obtained by contacting the Data Protection Officer as indicated above.

  4. You have the right to access your data, the right to request rectification, erasure or restriction of processing, as well as the right to personal data portability. To the extent that the processing of your personal data is based on the premise of a legitimate interest of the controller, you have the right to object to the processing of your personal data. You also have the right to lodge a complaint with a supervisory authority dealing with personal data protection. In order to exercise the above rights, please contact the controller or the Data Protection Officer. The contact details are indicated above.

  5. To the extent that the processing of your personal data is based on consent, you have the right to withdraw it. You can revoke your consent at any time by sending an e-mail to [email protected] or a letter to the administrator’s postal address.

  6. The Administrator ensures that he makes every effort to protect the privacy of persons using his services. To this end, it processes personal data in a manner that ensures adequate security of personal data, including protection against unauthorised or unlawful processing and accidental loss, destruction or damage, by means of appropriate technical or organisational measures.
